It's perfectly normal. She's still a baby. Most pups aren't reliable until around six months. Until then, keep taking her out regularly, keep treating her for going outside, and ignore any accidents inside. Make sure you're cleaning up with an enzymatic cleaner such as Simple Solution to remove any traces of smell that aren't detectable to our noses, but are to a dog.

A lot of normal cleaning sprays have ammonia in them which actually attracts them to wee on it. You need to get something like Simple solution which most pet shops sell. Also, even though she's 4.5 months old, she still need taking outside on a regular basis. I know when Harley was that age I thought that she knew that she had to go outside so took my eye off the ball for a bit. I soon learnt that I still needed to take her out on a regular basis.

Some dogs take a while to get the hang of toileting - some can take 9 months or more.
I think the more work you put in the quicker it is but some dogs are just slower.
I definitely second the use of non-ammonia cleaners. Hard floors are so much easier to clean, so until you are more secure with the house training, keeping your pup to hard easily cleaned floors is best.

I'm pretty sure PAH sell Simple Solution. If not, you can get it on Amazon :)
